If you are a Registered Nurse (RN) wishing to thrive in a positive, multidisciplinary team community environment, and with the freedom to offer individualized attention to frail seniors and adults with disabilities, please consider joining Family Bridges’ innovative Community Based Health Home (CBHH) as a Nurse Navigator.

Through the CBHH the Nurse Navigator will work with an interdisciplinary team of nurses, therapists, social workers and others to proactively coordinate care for older patients with complex medical conditions to address emerging needs while ensuring timely access to health and social services. The goal is to ensure comprehensive support.

This position holds the uniquely beneficial role of linking the daily services offered in our Hong Fook Adult Day Healthcare Center (ADHC) to the participant’s personal care physician, as well as to other resources to fully meet the participant’s needs. In addition, since our CBHH is located in a low income senior housing building, we are integrating health and wellness services for its residents. The Nurse Navigator will not only have a caseload of ADHC clients, but be responsible for providing nurse consultation and health and wellness education to building residents.

Whether a seasoned RN or a more recent graduate, the ideal candidate for this position is a person who desires to work within a cutting edge community based care setting to help low income patients avoid unnecessary hospitalizations and nursing facility placement.

This is a full time position in Oakland, Ca. Local Travel and a vehicle are required. BSN preferred. Salary: $72-$90K DOE plus benefits
