Community Health Worker / Care Coordinator

Family Bridges in Oakland is looking for a full-time Community Health Worker (CHW) to join our innovative Community Health Home (CHH) program. CHH is a Community Based Care Management Entity (CB-CME) serving primarily Medi-Cal members of the Alameda Alliance for Health and Anthem Blue Cross under their Alameda County Care Connect/Whole Person Care and Health Homes programs. The successful candidate thrives in a positive, team-driven environment, and enjoys working with frail older adults, persons with complex chronic medical conditions, disabilities or special needs, and/or people facing homelessness, substance abuse, mental health issues. We are looking for a trusted member of the community to: serve as a liaison and intermediary between health, behavioral health, social services and the community; facilitate access to services and resources (housing, transportation, healthcare); stabilize and ensure clients achieve a safe, stable and healthy living environment; and enhance the quality and cultural competence of services delivered to our clients.

JOB SUMMARY:  The Community Health Worker/Care Coordinator is a member of the multidisciplinary team responsible for supporting clients and addressing the social determinants that impact the clients’ health. Interactions with clients will be performed onsite at Family Bridges and/or in clients’ current place of residence, including hospitals, skilled nursing or other care facilities, homeless residence or encampments, and temporary or transitional housing.

MAJOR DUTIES AND RESPONSIBILITIES:

  • Outreach to and engage eligible CHH clients.
  • Conduct risk assessments and intake interviews with clients and/or families.
  • Advocate for clients with health care professionals by meeting clients in various settings, including but not limited to: medical offices, hospitals, emergency rooms and skilled nursing facilities.
  • Contribute toward developing client care plans that reflect individual clients’ needs and connect clients to appropriate resources as appropriate.
  • Assist with coordinating client care and accompany client as appropriate e.g. medical appointment reminders, transportation arrangements, referrals, and preventive care education and/or follow-up care adherence.
  • Connect clients with needed social service and support resources (housing, transportation, healthcare), including assisting with necessary enrollment processes and forms as needed. This may also include accompaniment to social security offices, department of motor vehicles, housing agencies and other government agencies
  • Collect data, track processes and outcomes, participate in activities to design and implement standard policies and protocols, and prepare case reports.
  • Document encounters, assessments and progress notes, and plan interventions.
  • Provide health promotion materials, and health promotion and self-management training.
  • Support and perform administrative duties.
  • Represent and promote Family Bridges’ Community Health Home through CB-CME network, Alameda County Care Connect, and other associated meetings, group presentations, and trainings, as well as one-on-one interactions with clients.
  • Form and foster relationships and partnerships with health and behavioral health care providers, and housing/supportive agencies and providers.

REQUIRED QUALIFICATIONS:

  • Work experience in community based health care service capacity.
  • English language proficiency.
  • Valid California driver’s license.
  • Up-to-date medical physical examination and immunizations.

PREFERRED/DESIRED QUALIFICATIONS:

  • Experience working in a community outreach program performing duties such as intake, peer counseling, assessments, program screening, placement and referral.
  • Experience working with frail older adults, persons with disabilities or special needs, and/or people facing homelessness, substance abuse, mental health issues, and/or complex chronic medical conditions.
  • Experience integrating motivational interviewing and/or trauma informed care practices.
  • Experience using a Salesforce database/CRM platform.
  • Familiarity with Alameda County Care Connect/Whole Person Care and Health Home programs.
  • Bachelor’s Degree in a health, social science or related field such as psychology, counseling, social work, medical social work preferred.
  • Bilingual Cantonese, Mandarin, or Vietnamese a plus.

  • Ability to use smartphone and/or tablet (e.g. iPad).
  • Ability to travel locally multiple times a day.
  • Ability to perform responsibilities in various environments multiple times a day.
  • To lift a minimum of 20 lbs.
  • Vision 20/20 with correction.

SALARY RANGE:  $38k-$45k plus benefits, depends on experience.
