COMMITMENT (HOURS): 40 hours per week

STATUS: Non-Exempt (3 months orientation period)

Family Bridges in Oakland is looking for a full-time Social Worker Assistant (SWA) wishing to thrive in a positive, multidisciplinary team community environment, and provide comprehensive support for adults with complex medical conditions, frail seniors, and/or adults facing homelessness to join our innovative Community Health Home (CHH). We are looking for a trusted member of the community to serve as a liaison and intermediary between health, behavioral health, social services and the community to facilitate access to services and improve the quality and cultural competence of services delivered to our patients.

JOB SUMMARY:  The Social Worker Assistant will work with an multidisciplinary team (RN, MD, MSWs, LCSWs, and SWAs) to support patients and address the social determinants that impact the patients’ health by linking the patient with resources (housing, transportation, healthcare). The goal of the innovative CHH is to support and empower identified high risk-high needs patients to improve their health outcomes.

MAJOR DUTIES AND RESPONSIBILITIES:

  • Conduct risk assessments or intake interviews with patients and/or families; assess patient’s level of functioning, environment, appropriateness and adequacy of support system related to illness and ability to cope.
  • Screen for any barriers to care such as substance abuse, neglect or housing.
  • Perform assessment/interviewing/screening responsibilities onsite at Family Bridges facilities and/or in patients’ place of residence, including homeless encampments, and nursing and other care facilities.
  • Advocate for patients by meeting patients in various settings: medical offices, specialists, hospitals and patients’ homes.
  • Assist with developing patient care plans that reflect individual patients’ needs and connect patients to appropriate resources as appropriate.
  • Assist with coordinating patient care, e.g. transportation arrangements, medical appointment reminders, referrals, and preventive care education and/or follow-up care adherence.
  • Work with patients facing homelessness, substance abuse, mental health issues, and/or complex chronic medical conditions.
  • Prepare case reports, document assessments and progress notes, plan interventions.
  • Promote Family Bridges’ Community Health Home through group presentations and one-on-one interactions in various settings, including homeless encampments

REQUIRED QUALIFICATIONS:

  • Work experience in community based health care service capacity.
  • English language proficiency.
  • Valid California driver’s license.

PREFERRED/DESIRED QUALIFICATIONS: 

  • Experience working in a community outreach program performing duties such as intake, peer counseling, assessments, program screening, placement and referral.
  • Experience working with frail seniors, disabled, and/or populations facing homelessness, substance abuse, mental health issues, and/or complex chronic medical conditions.
  • Bachelor’s Degree in a health, social science or related field such as psychology, counseling, social work, medical social work preferred.
  • Bilingual Cantonese, Mandarin, or Vietnamese a plus.

ADA REQUIREMENTS: 

  • Ability to communicate clearly with others.
  • Ability to see and hear in order to interact with others.
  • Ability to use a computer.
  • To lift a minimum of 20 lbs.
  • Vision 20/20 with correction.

SALARY RANGE:  $38k-$45k, depends on experience.
